The back cover says: "Recorded live at the Civic Auditorium, Milwaukee, WI 07/03/94"
There is no "Civic Auditorium" in Milwaukee, and I can find no information about any Candlebox performances on either July 03, 1994 or July 30, 1994 (which this is sometimes written as). There are none on March 7, 1994 either, I checked incase this was a European bootleg.
Summerfest was at the Marcus Ampatheater from June 31 to July 10, 1994. So, that could possibly be where this was recorded. Candlebox doesn't appear on the billing, but Metallica performed there on July 1, 1994, and Candlebox was on tour with Metallica during June and July of 1994
